How it was done: I had a friend do a color separation on her computer and got films made for screen printing and had a poster artist friend print these in his basement workshop. A printer friend scored them so they would fold with a print on the spine. These came out looking really cool, but were really expensive since I hired out most of the work. 
Where it was sold: My dog got sick and needed an operation so I had to sell these through the mail to raise cash to pay the vetrinarian.
